Medical Case Manager, HIV Services

The Medical Case Manager, HIV Services role is onsite in Boston, MA. Pay is $23–$35.83 per hour. The position supports adult clients with HIV/AIDS in the Codman community and reports to the Manager of HIV Services. The case manager coordinates medical and social-service support, advocates for clients’ clinical and psychosocial needs, and connects them with community resources. Through patient visits, referrals, education, and collaboration with care providers, the role contributes to ongoing and emerging patient needs.

What you'll do

• Provide case management to HIV-positive patients
• support new-patient intake and onboarding
• reinforce self-determination, medication adherence, and patient engagement
• collaborate with physicians, nurses, and other case managers
• refer clients to community agencies for social determinants of health needs
• conduct monthly office or telephone visits and update care plans
• maintain activity and variance reports
• attend requested team meetings and complete paperwork on time.
