HIV Program Case Manager

What you'll do

• Provide intake, assessment, care-plan development, and follow-up for Ryan White Part B and C patients.
• Coordinate care, including home visits, and advocate for specialty referrals, housing, appointments, ADAP, prescription regimens, transportation, financial and social services, pharmaceutical resources, and support groups.
• Conduct HIV and Hepatitis C testing in-house and through outreach, including social networking to identify new diagnoses.
• Travel to rural areas within the service area to provide case management.
