The Community Liaison role is an onsite, full-time position based in Baltimore, MD, with regular travel to facilities and community partners throughout Maryland. Compensation is $28–$30 per hour. The role supports Nurse Practitioner On Call’s effort to expand access to psychiatric medication management and therapy in long-term care settings. The liaison develops facility partnerships and referral relationships, guiding opportunities from outreach through agreement and service launch. The role contributes to sustainable growth and helps bring on-site care programs to residents in partner facilities.

What you'll do

• Identify and pursue long-term care facility opportunities
• build relationships with facility decision-makers
• present medication management and therapy services
• guide opportunities through proposals, approved agreements, and service launch
• develop referral networks
• conduct facility visits, outreach, presentations, and networking
• maintain prospect and contract records
• report progress to leadership
• coordinate with intake, clinical, and operational teams
• support onboarding, retention, and referral follow-through
• travel regularly throughout Maryland.
