What you'll do

• Conduct intake and initiate required case forms
• complete program chart quality reviews
• build rapport and provide empathetic support
• assess safety and options for addressing immediate threats
• maintain community resource contacts
• develop action plans with participants
• provide information, referrals, and transportation
• attend case conferences and staff meetings
• maintain confidentiality and timely case records.
