What you'll do

• Provide initial assessments and behavioral interventions for patients referred by primary care providers
• use brief cognitive behavioral therapy and evidence-based practices
• support patient self-management
• provide feedback to providers
• conduct short-term follow-ups and crisis intervention as needed
• use screening data
• facilitate referrals
• and help develop relevant clinic services.
