The Outside Sales Representative (Outreach Specialist) role is onsite in Modesto, CA; compensation is $75,000 per year, with base salary plus bonus. WelbeHealth provides life-extending care through its PACE model, helping vulnerable seniors live safely in their homes rather than moving to skilled nursing facilities. As part of the Marketing, Outreach, & Enrollment team, the role builds community relationships, generates qualified referrals, and guides prospective participants toward enrollment. It coordinates outreach, develops monthly marketing plans with the MOE Director, and educates referral sources about WelbeHealth services.
