What you'll do

• Connect PACE participants with the care system and serve as a liaison to the care team
• facilitate communication among providers, families, participants, and caregivers
• make customer-service calls about appointments
• support grievances and service recovery through resolution
• conduct follow-up check-ins
• assist with participant-experience projects and surveys
• attend required meetings and committees.
