Registered Nurse Case Manager – Hospice

The Registered Nurse Case Manager – Hospice role is field-based in Fort Wayne, IN, with a Monday–Friday schedule from 8:30 a.m. to 5:00 p.m. Pay is $75,000–$90,000 per year. Moments Hospice provides comfort-focused care and support to patients and families at the end of life. The nurse leads clinical care for an assigned caseload by assessing needs, managing symptoms, and coordinating individualized plans with patients, families, physicians, and the interdisciplinary team. The role also educates caregivers and supports comfort, dignity, and continuity of care.

What you'll do

• Complete initial and ongoing assessments of physical, emotional, and psychosocial needs
• develop and update individualized care plans with patients and families
• provide skilled nursing care, pain and symptom management, medication administration, and comfort measures
• educate patients and caregivers
• coordinate care and serve as liaison among patients, families, physicians, and the care team
• participate in interdisciplinary group meetings.
