Social Service Designee, Skilled Nursing

What you'll do

• Complete psychosocial assessments
• participate in the MDS process and person-centered care planning
• support residents and families
• coordinate family meetings and discharge plans
• arrange community resources and post-discharge services
• educate residents and families about rights
• assist with grievances and advocacy access
• coordinate behavioral health referrals
• maintain compliant electronic health records
• participate in QAPI and survey preparedness.
