What you'll do

• Identify and arrange external services
• schedule discharge meetings and coordinate home visits, home care, and therapy
• lead timely discharge planning meetings for newly admitted residents
• assess support systems and home situations
• support bed management
• assist residents with social issues, conflict resolution, and care coordination
• conduct customer satisfaction surveys.
