Wound Care Skin Management Nurse

The Wound Care Skin Management Nurse role is onsite in Irvine, KY. Compensation is not specified. The nurse supports the skin management program at Irvine Nursing and Rehabilitation Center. The role assesses residents’ skin, documents findings, and coordinates care plans and treatments to prevent avoidable skin breakdown and support healing. Communication with physicians, residents, families, and the interdisciplinary team helps ensure appropriate interventions and follow-up.

What you'll do

• Assess residents’ skin weekly, identify risks, and document findings
• measure skin areas weekly
• initiate and update skin-related care plans
• report skin problems to physicians and follow up on orders
• perform skin treatments as assigned under physician orders and professional standards
• communicate resident status to physicians, residents, families, nurses, interdisciplinary team members, and facility management
• chair weekly Skin Management meetings
• monitor equipment use and maintain records
• provide weekly skin logs and education
• participate in quality improvement activities.
