What you'll do

• Participate in developing, monitoring, and revising program plans
• identify and coordinate services for individuals and families
• complete documentation and reports
• advocate for individuals served by community agencies
• schedule and attend interdisciplinary meetings
• identify service-delivery barriers and use culturally sensitive strategies
• rotate officer-of-the-day duties with Service Coordinators
• provide case coverage and emergency on-call services when required.
