Admission Registered Nurse

The Admission Registered Nurse role is onsite in Seattle, WA, full-time, with a stated annual salary range of $104,000–$114,400. The nurse reports to the Clinical Director and supports hospice care for patients and families in their homes and communities. The nurse assesses patients and families, develops and updates individualized plans of care, provides nursing interventions and education, and coordinates with physicians, caregivers, and the interdisciplinary group. The role helps ensure hospice services respond to changing patient needs and support comfort and continuity of care.

What you'll do

• Complete initial and ongoing patient and family assessments
• provide nursing care using the nursing process
• develop and revise plans of care with patients and families
• administer prescribed medications and treatments
• provide counseling and health instructions
• assist with activities of daily living
• coordinate assigned caseloads when acting as case manager
• maintain clinical notes
• communicate patient changes to physicians
• coordinate with community health contacts and the interdisciplinary group
• help arrange equipment and services
• participate in quality improvement activities.
