MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty.

Experience

MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty required. Board certification or eligibility preferred. Must have a State Medical License and DEA number.
