Description
Remote RPM Alerts Care Coordinator (LPN/LVN). TimeDoc Health is seeking compassionate and patient-focused LPN to join our RPM Alerts Team and help patients with chronic conditions stay healthier and out of the hospital. Respond to Remote Patient Monitoring (RPM) alerts and conduct patient outreach. Support patients with chronic conditions through Chronic Care Management (CCM) and RPM services.
