What you'll do

• Conduct clinical and mental health needs screenings and assessments
• develop individualized referrals to therapy partners
• maintain partner relationships and track referral outcomes
• design and facilitate therapeutic groups
• consult with program staff and leadership on complex needs
• support staff training in trauma-informed care
• advise on response, referral, and escalation for safety concerns
• maintain clinical records
• contribute wellness data to grant reporting.
