Advanced Practice Pharmacist

The Advanced Practice Pharmacist role is onsite in Ontario, CA. Compensation is $150,000–$170,000 per year. Akido combines technology with a medical network to provide primary and specialty care and expand access to healthcare. The pharmacist provides comprehensive medication management across primary and specialty care, working with physicians, advanced practice providers, nurses, and care teams. The role optimizes medication therapy, supports chronic disease management and patient education, and contributes to medication safety, adherence, quality measures, and population health outcomes.

What you'll do

• Provide medication management for patients with chronic and complex conditions
• evaluate and optimize medication regimens
• initiate, modify, monitor, and discontinue medications under applicable regulations and collaborative practice agreements
• conduct assessments and medication reviews
• monitor effectiveness, adverse reactions, adherence, and safety
• collaborate on treatment plans
• counsel and educate patients
• document in the EHR
• participate in quality improvement and population health programs.

Why this role
