What you'll do
- Provide outpatient endocrine care, including chronic disease management.
- Manage Type I & Type II diabetes and other endocrine-related conditions.
- Deliver follow-up visits, urgent add-on visits, and work within a care-team pod model.
- Collaborate closely with physicians, RNs, diabetes educators, and clinical support staff.
- Provide patient education on diet, exercise, medications, and long-term disease management.
