Housing Case Manager

What you'll do

• Assess needs and develop case plans
• coordinate housing placement and community-resource connections
• maintain HUD compliance through navigation, lease-up, and recertification
• coordinate with housing agents and property managers
• identify housing options and facilitate lease signing
• support lease compliance
• coordinate health and behavioral health appointments
• respond to occasional mental health crises
• assist with daily living skills
• provide flexible support and skills training
• maintain case records, HMIS and ECO data, and reports
• monitor rental payments and eviction prevention plans.
