What you'll do

• Conduct hospice and palliative care visits
• evaluate patients for pain and symptom management, hospice eligibility, and recertification
• counsel patients and families on care options and advance care planning
• order tests, initiate treatment, conduct face-to-face visits as needed, serve as attending physician when appropriate, coordinate with interdisciplinary teams, manage medications, and complete clinical documentation. Participate in an on-call rotation and provide 24/7 availability when assigned.
