What you'll do

• Manage hospice plans of care from referral through discharge
• assess patients and families

• administer prescribed medications and treatments
• document responses and clinical notes
• teach patients and families about pain and symptom management
• coordinate with the care team and physicians
• complete discharges and handoffs
• participate in interdisciplinary meetings and quality improvement.
