What you'll do

• Provide community transportation and support participation in jobs, social activities, and appointments
• assist with personal care as needed
• teach daily living skills aligned with participant goals
• administer and document medications
• provide light housekeeping, meal preparation, and grocery shopping
• follow individual service plans and safety protocols
• report incidents or neglect
• document participant progress
• attend required training and team meetings
• use crisis-management training and backup resources when needed.

Why this role
