WellPower provides collaborative behavioral health care and comprehensive support to help people achieve lasting well-being. The role provides psychological and behavioral case management across levels of service intensity, helping people maintain or improve their ability to function in the community. It supports recovery by addressing basic needs, connecting people with services and resources, and documenting clinical interactions.

What you'll do

• Assess, monitor, and follow up on people’s ability to meet basic needs
• assist with benefits applications and protective payee arrangements
• arrange or provide transportation in a personal vehicle
• provide psychosocial services, daily-living-skills guidance, and support with goals and service planning
• connect people with housing, employment, community resources, and natural supports
• support treatment engagement
• maintain clinical documentation and a trauma-informed environment.
