What you'll do

• Provide intensive case management and rehabilitative services
• coordinate with an interdisciplinary treatment team
• maintain a caseload and timely documentation
• provide crisis services, including after-hours response for ACT clients on a team rotation
• assess needs and support individualized treatment goals
• follow up on referrals
• connect clients to services
• and maintain required assessments and compliance records.

Why this role
