The counselor provides therapeutic counseling to patients and families, assesses needs using clinical input, and develops intervention or care plans. The role also conducts group therapy when appropriate, makes community referrals, documents contacts, and responds to patients in crisis through telephone triage and immediate intervention when necessary.

What you'll do

• Provide therapeutic counseling to patients and families
• assess needs using clinical input from providers and others familiar with the patient
• develop intervention, care, or assistance plans
• conduct group therapy when appropriate
• refer patients to community resources
• document contacts
• perform telephone triage for patients in crisis and provide immediate intervention when necessary
• use EPIC for documentation.
