What you'll do

• Guide interdisciplinary teams in comprehensive hospice care
• coordinate nurses, physicians, social workers, spiritual care providers, and other care professionals
• lead continuous improvement and support clinical compliance
• oversee caseload management, productivity, resource use, and service quality
• coach and mentor team members
• support initiatives to improve outcomes and patient experiences.

Why this role

Requires a bachelor's degree in nursing or a related health or human services field, or equivalent experience, and at least three years of hospice clinical experience. Clinical management experience and hospice triage or after-hours operations experience are preferred.
