The Wellness Nurse (LPN/LVN) role is onsite in Jacksonville, FL. Compensation is $28.65–$38.25 per hour. At Sunrise Senior Living, the role supports resident health and the organization’s focus on quality of life for seniors. The nurse monitors residents’ health and care needs, supports safe medication practices, and helps maintain clinical compliance. Through assessments, communication, documentation, and care-team support, the role contributes to resident well-being and clinical quality.

What you'll do

• Conduct wellness assessments and monitor changes in resident health
• support medication management and perform MAR/TAR audits
• communicate with families, physicians, and care teams
• monitor incidents and falls and follow up on care
• maintain documentation and regulatory compliance
• support infection control and clinical quality initiatives
• train and support care team members.
