Per Diem Physician - Post-Acute & Senior Care

Provides direct patient care in multiple facilities across the aging continuum to include skilled nursing, rehab, personal care/assisted living, independent living and clinic. Interacts with facility leadership, hospital discharge team, care management and other specialties to drive optimal quality care with aligned goals. Collaborates with and supervises Advanced Practice Providers.
