Community Health Worker

What you'll do

• Conduct outreach and build rapport in community settings
• complete needs assessments, social-determinants-of-health screenings, and care plans
• support access to healthcare, housing, mental health, social services, and community programs
• provide crisis support within scope and make referrals
• assist with appointments and applications
• document outreach and care activities
• collaborate with providers and community partners
• provide culturally sensitive health and wellness education.
