What you'll do

• Educate staff about dietary needs and end-of-life interventions
• assist physicians and nursing personnel in determining patient dietary needs
• develop nutritional plans of care with the interdisciplinary group based on comprehensive assessments, preferences, and choices
• provide dietary assessments and counseling
• support agency staff as requested
• participate in QAPI activities as appropriate
• and maintain clinical and agency records.
