What you'll do

• Assess patients and develop individualized home health care plans
• administer medications, wound care, IV therapy, and other skilled nursing treatments
• monitor progress and coordinate care-plan changes with physicians
• educate patients and families on disease management, medications, and home safety
• document care and visits
• collaborate with therapists, social workers, and physicians
• respond to changes in patient condition and escalate care when appropriate.
