What you'll do

• Map and prioritize healthcare and community accounts
• build relationships with hospitals, physicians, rehabilitation centers, skilled nursing and senior living communities, discharge planners, and case managers
• maintain territory call plans
• educate referral partners
• conduct presentations and outreach
• follow up on referrals
• track activity in the CRM
• attend community events
• partner with the clinical team to help patients access home care.
