What you'll do

• Train patients and care partners in peritoneal dialysis or hemodialysis
• conduct initial and at least annual home visits and additional assessments as needed
• communicate assessments and monitoring to physicians and team members
• document nursing services in the EMR
• support infection control and emergency preparedness
• manage supplies
• obtain and communicate lab results
• review medication profiles and administer ordered medications
• provide rotational on-call nursing services nights and weekends.
