What you'll do

• Provide lead-level consultation and guidance to case management and direct service staff
• coordinate crisis follow-up and case management services
• support timely referrals and transitions from crisis intervention, emergency services, and psychiatric hospitalization to ongoing care
• provide direct case management and follow-up, including outreach, assessment, referrals, linkage, and advocacy
• collaborate with clients, families, clinicians, schools, hospitals, and community providers
• support staff training and coaching
• document services and promote client-centered, culturally responsive, recovery-oriented practices.

Why this role
