What you'll do

• Manage a caseload of hospice patients across the Denver metro area
• provide personalized care and pain and symptom management
• assess patients and develop individualized plans with the interdisciplinary team
• educate patients and families
• coordinate with physicians, facility staff, and care team
• direct hospice aides and care staff
• complete timely documentation
• participate in the on-call rotation.
