What you'll do

• Conduct resident outreach and follow-up, primarily by phone or virtually, coordinating with onsite SNF staff
• administer and track PHQ-9, GAD-7, and other screening tools
• maintain a behavioral health registry and monitor outcomes
• provide brief evidence-based interventions
• collaborate with nursing staff, primary care providers, and care teams
• present cases at weekly psychiatric reviews and implement recommendations
• document time and services for CoCM billing
• maintain accurate, timely EMR documentation.
