The nurse provides after-hours home care and hospice support, responding to calls, assessing needs, managing symptoms, and delivering direct care. What you'll do

• Answer client and family calls to determine hospice needs
• assess clients and their current and previous illnesses
• provide weekend and evening follow-up
• follow and contribute to the care plan
• administer prescribed medications and treatments
• counsel clients and families
• provide health care instructions
• support discharge planning
• prepare clinical notes and update physicians about changes
• coordinate with community health partners
• complete care-plan documentation by 10 a.m. the next day
• manage supplies and medication orders
• organize travel and document mileage.
