What you'll do

• Assess patients during visits and document observations
• take vital signs and review medications
• administer prescribed medication
• dress wounds and monitor healing
• educate patients and families on home health strategies
• coordinate with occupational and physical therapists, physicians, and other care-plan participants
• provide palliative care as needed
• recommend devices or tools that may improve quality of life
• and respond to patient concerns.
