What you'll do
- Review referrals, intake updates, and clinical reports to identify hospice-appropriate patients
- coordinate evaluations and admissions with hospitals, skilled nursing facilities, senior living communities, physicians, and discharge planners
- complete admissions, eligibility assessments, consents, patient and family education, and start-of-care documentation
- provide visit nurse and case management support as needed
- collaborate with interdisciplinary team members
- build referral partner relationships
- maintain accurate EMR documentation and regulatory compliance
- travel throughout the service area.

Why this role
