Social Services Designee

Job overview

The Social Services Designee role is onsite in Ripon, WI, at Whispering Pines. Compensation is $50,000–$60,000 per year. Whispering Pines is a skilled nursing provider within the Eden Senior Care network, which supports residents and their families. The designee supports resident care by gathering social, emotional, and financial information, coordinating discharge planning, and working with residents, families, healthcare providers, and community resources. The role also helps coordinate admissions, care planning, resident advocacy, and transitions to home or a lower level of care.

What you'll do

• Conduct resident interviews
• assist with discharge planning and coordinate with healthcare providers, families, and community resources
• document interviews, calls, and care conferences
• communicate with interdisciplinary team members
• support referral-to-admission coordination
• communicate with insurers about prior authorization
• assist with admission paperwork and Medicaid applications as needed
• support resident assessments and written care plans
• attend and lead behavior meetings
• complete progress notes
• provide counseling or referrals
• report concerns and grievances
• advocate for resident rights
• participate in QAPI and daily stand-up meetings
• coordinate safe discharges
• issue denial letters according to state regulation.

Why this role
