What you'll do

• Assess patients’ physical, functional, psychosocial, and environmental needs
• determine hospice admission and recertification eligibility
• develop and document care plans
• educate patients and families
• coordinate services with the interdisciplinary group
• direct hospice aides and LPN/LVNs
• document progress
• communicate changes to physicians and other personnel
• and provide on-call service as assigned.
