What you'll do

• Support individuals with budgeting and money management, hygiene and household tasks, activities of daily living and instrumental activities of daily living, and community access
• maintain health and safety
• complete daily progress notes and quarterly reports
• follow individualized service plans
• attend training and biweekly supervision
• maintain confidentiality
• facilitate conflict resolution as needed
• attend physician appointments, SIS assessments, and annual reviews related to individual goals.
