Memory Care Activity Coordinator

Job overview

The Memory Care Activity Coordinator works onsite in Deerfield, IL. Compensation is $17.50 per hour. Elevate Care provides post-acute care, skilled nursing, rehabilitation, and post-hospital recovery services. The coordinator plans, implements, and evaluates resident activities in accordance with assessments and care plans, supporting residents’ physical, social, intellectual, emotional, and spiritual well-being.

What you'll do

• Plan and support activity programs
• observe and document resident attendance, mood, behavior, and participation
• maintain activity progress notes
• coordinate with facility departments
• prepare and clean activity areas
• follow resident care plans
• support residents during scheduled activities and in-room visits.
