Care Coordinator

The role oversees coordinated, individualized resident care and helps maintain clinical quality and compliance. By guiding nursing staff, monitoring care plans and medication documentation, and coordinating with residents, families, physicians, and the Administrator, the Care Coordinator supports safe and consistent services.

What you'll do

• Supervise implementation and coordination of resident care and services
• direct nursing staff clinical responsibilities
• evaluate residents and report changes
• review and update care interventions according to physician orders and service plans
• oversee medication management documentation in EMR/MAR
• ensure accurate regulatory documentation
• coordinate admissions, transfers, and discharges
• supervise licensed nurses, aides, nursing assistants, and medication technicians
• follow up on incidents and notify physicians, families, and the Administrator when residents are ill or injured
• provide direct nursing care within scope of practice.
