What you'll do
- Manage referrals for ICF/IID, DDA group homes, and community services.
- Coordinate admissions, documentation, and transition planning.
- Maintain relationships with DHHS, Tailored Plans, and LME/MCOs.
- Ensure compliance with NC Medicaid and DHHS standards.
- Participate in Person-Centered Planning and outreach.
