What you'll do
- Complete and coordinate accurate, timely MDS assessments
- develop, review, and update individualized care plans
- coordinate care-plan meetings with residents, families, and the interdisciplinary team
- monitor documentation and review clinical records for accuracy and compliance
- track Medicare and managed-care assessment schedules
- participate in utilization review, Triple Check, quality improvement, compliance activities, audits, and surveys
- educate nursing staff on documentation requirements and changes in resident conditions
- communicate with physicians, nursing leadership, therapy, residents, and families
- maintain knowledge of MDS, RAI, PDPM, Medicare, Medicaid, and applicable regulations.
