What you'll do

• Coordinate care across providers and maintain individualized service plans
• complete progress notes, assessments, and other required documentation
• provide individual and group community psychiatric supportive treatment and therapeutic behavioral services
• support treatment planning, interventions, social skills, daily functioning, and crisis prevention
• connect clients with resources and advocacy
• transport clients as needed
• maintain confidentiality and complete required training.
