What you'll do

• Provide outpatient primary care to seniors with chronic diseases
• see approximately 18–20 patients per day with a maximum panel of 450
• conduct visits within the Monday–Friday clinic schedule of 7:30 a.m.–5:00 p.m., with the last patient scheduled at 4:15 p.m.
• use included administrative time
• work with a dedicated medical assistant, scheduler, and referral specialist. Inpatient call or weekend coverage is optional and may provide additional compensation.
