Registered Nurse (RN) Case Manager, Care Management

The Registered Nurse (RN) Case Manager, Care Management role is onsite in Grand Junction, CO. Compensation is $2,074 per hour as stated in the posting. This is an 8-week, full-time assignment with 36 guaranteed hours per week and five hours of coverage on one Saturday per month, offset by five hours off during the week. The nurse coordinates, monitors, and evaluates patient care to support appropriate, effective, and cost-efficient services across the healthcare continuum, working with patients, providers, payers, and interdisciplinary teams.

• Assess and coordinate patient care needs

• coordinate discharge planning and levels of care
• collaborate with physicians, patients, families, staff, and payers
• monitor resource utilization
• document case management activities
• educate physicians and staff on utilization and level-of-care issues
• participate in departmental and interdisciplinary meetings
• support quality, operational, and financial outcomes.

Why this role

Current unrestricted Colorado RN license, BLS certification, and at least 3 years of clinical hospital experience required. Bachelor’s degree in nursing required; master’s degree and case management certification preferred. 36 guaranteed hours per week.
