What you'll do

• Review patients’ medical, functional, and psychosocial needs
• assist with HEDIS quality measures
• reinforce disease self-management and preventive care guidance
• help patients access referrals, labs, radiology, prescriptions, and community resources
• audit charts and submit required information to health plans
• answer and triage patient calls
• communicate care gaps and changes in condition to clinical teams and physicians.
